Occupational Medical Services and Pricing:

• Comprehensive Drug Testing- With Alcohol $55.00/ With-Out Alcohol $50.00 If chain of custody is provided, $20.00 handling fee
• Treatment of Acute Injuries - $147.00 on up (with Insurance). $80.00 (cash pay) for the 1st visit, $50.00 any time after the 1st visit. Labs and tests may be extra.
• Pre-Employment Physical - $35.00 and up
• General Pay - $80.00 for cash pay. With insurance, depends on insurance contract rate
• Laboratory Services - Some labs are available in-house and some are sent out. Price pending on lab services
• Vision Test - Included in price of physical
• Electrocardiogram - $69.00 in house plus the office visit
• NON-DOT Physicals - $75.00 ($65.00 for physical /$10.00 for urinalyses) = $75.00 total
• Private Medical Care - Cash pay patients $80.00 for the 1st visit, after the 1st visit $50.00 an up. Labs and tests may be extra.
• TB testing - $20.00
• Flu shots - $20.00
• School Physicals - $25.00 (for basic physical)
• On-site X-Ray testing - Pricing determined per X-Ray
• Urinalysis testing - $10.00 plus price of office visit (Cultures are extra)

Whether you need a simple routine exam or you are looking for on-going treatment, our locally owned office will take great care of you. We accept PPOs, Medicare and some HMOs. Pricing will be determined by the insurance contract rate.
